Preschool & 4K Required Forms – Peace Lutheran Church

Welcome to Peace Lutheran Preschool and for many of you “Thank you for returning to Peace Preschool next fall”! This page contains important information and instruction for the paperwork required for enrollment in the 2022-23 school year.

Completed paperwork can be returned by:

Using the “Drop Box” located between the colorful Little Library and Bumble Bee playground.
Mailing to Peace Lutheran Preschool, 701 South Century Avenue, Waunakee 53597.
Dropping off in the church office. Please call and make sure the office will be open as summer office hours are limited (608 849 7732).

Completed paperwork is due by Friday, July 22, 2022. Paperwork turned in after that date will be assessed a $30 late fee. Exception to the deadline is the “Child Health Form- DCF” which requires a medical provider signature. This form is due by August 31st, 2022 to allow time for medical appointments.
